Assumption of Risk:

I understand that participating in Reformer Pilates classes involves certain inherent risks, including but not limited to, muscle strains, sprains, and other injuries. I voluntarily assume all risks associated with participation, including those arising from the negligence of instructors, staff, or fellow participants.

Health and Fitness:

I confirm that I am in good physical condition and have no medical conditions or injuries that would prevent me from safely participating in Reformer Pilates classes. I agree to inform the instructor of any changes to my health or medical status that may affect my ability to participate safely.

Responsibilities:

I agree to follow all instructions provided by the instructor during classes and to ask for clarification if I do not understand any exercises or techniques. I understand that I am responsible for monitoring my own physical condition during classes and for stopping any activity that causes discomfort or pain.

Release of Liability:

I hereby release, waive, discharge, and covenant not to sue Extend Studio, its owners, instructors, employees, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me during or as a result of participating in Reformer Pilates classes.

Indemnification:

I agree to indemnify and hold harmless Extend Studio, its owners, instructors, employees, and affiliates from any and all claims, liabilities, damages, costs, or expenses, including attorney fees, arising out of or related to my participation in Reformer Pilates classes.

Photography and Video:

I consent to the use of any photographs or videos taken of me during Reformer Pilates classes and my child for promotional or educational purposes by Extend Studio. I understand that my name will not be associated with any such materials without my explicit consent.

Agreement to Terms:

I acknowledge that I have read, understand, and agree to abide by all the terms and conditions of this Reformer Pilates Waiver Contract. I understand that this agreement is legally binding and that I am waiving certain legal rights.
